* Project Location: 27 East Main Street, Gowanda NY 14070 — Gowanda Village Hail Roof Abatement
Asbestos Visual Clearance Inspections per NYS ICR-56 for Roof Abatement work
« Scope of Work: AMD will perform Exterior Asbestos Visual Clearance inspections for Asbestos Roof Abatement in
conjunction with Paramount Roofing for Asbestos Built up roofing — Applied directly to wood roof deck beneath roof
membrane and polyisocyanurate insulation board per Stohl Environmentals 6/3/26 Hazardous Materiais Inspection
Report. There is approximately 3,052 sq. ft. of asbestos roofing located an Upper roof system and approximately 1,730
sq, ft. of asbestos roofing on the Lower roof system.
« AMD Environmental, Inc, (NYS DOL # 56177) will conduct asbestos inspections in accordance with guidelines and
techniques identified in NY Code Rule 56. PCM Air sample analysis will be conducted at AMD Environmental Consultants’
laboratory, a NYS ELAP approved facility in Buffalo, New York (ID: 11108) if applicable to project. A final close-out report
will be issued upon project completion containing all sample analysis, chains of custody, sample location maps, and visual
inspection forms pertaining to the project.

Exterior Asbestos Visual Clearance Inspections for Asbestos Roof Abatement: $ 3,500.00
NYS / EPA Licensed Asbestos Inspector:
$350.00 per site visit for included up to ten site visits per Paramounts 10/07/26 estimated duration
Asbestos Project Manager Rate: $ 450.00
$75.00/ Hour for estimated 6 hours for project coordination, oversight and compliance
Final Close Out Report / NYS Dept. of Labor Compliance & Document Submissions: $ 300.00
ESTIMATED TOTAL COST: $ 4,250.00
CARD PAYMENT CONVENIENCE FEE: A fee of 3% will be added to all card payments. To keep our prices fair, we are passing this cost onto
card-paying customers. This fee can be avoided by paying with cash ar check.
Note: If total days on-site exceeds the estimated days, the total cost will reflect the additional days at the Standard Day Rate. Over-lime is not
included in this quote and associated fee may be negotiated if deemed necessary for job completion.
