PAY REQUEST NO. 5, 6 AND 7 (FINAL)
Dear Mayor Nephew:
We are enclosing Payment Requests Numbers 5, 6 and 7 (Final) from Quackenbush Co., Inc. on the above
referenced project in the total combined amount of $16,606.09. Pay Request No. 7 is the Final Pay Request
and is paying for all retainage. Also enclosed are the following closeoutdocuments:
¢ Certified Payroll with each pay request.
¢ Change Order No, 1, which is a $10,000 deduct change order for contingency allowance that was not
used.
Consent of Surety to Final Payment
Contractor's Affidavit of Payment of Debts and Claims
Contractor's Affidavit of Release of-Liens
Major Subcontractors’ Affidavits of Release of Liens
Wendel has checked all items, find them to be in order, and would recommend payment of all three pay
requests.
