Transit Related Title VI Complaints, Investigations and Lawsuits The Town of Hamburg maintains a log of all Title VI complaints, investigations, and lawsuits pertaining to its transit-related activities since the last Title VI plan update. Title VI complaints must be logged Reporting Period: Oct 2023- Oct 2024 at least annually. You may use this form for all three years, but you will Nov 2024- Oct 2025 need to fill out each reporting year separately. This should be for the Nov 2025- Current PREVIOUS three years. Note: do not include the complainant’s name. Use of a case Check One: number is preferrable.
