Re: Electric Service Proposal 31203402
2800 Lakeview Rd, Hamburg, NY 14075

Enclosed please find an Electric Service Proposal for installing new pole/transformer,
replacing 2 poles and installing 200’ overhead single-phase primary wire at 2800
Lakeview Rd, Hamburg, NY.

This project falls under PSC No. 220 Tariff Rule 28.2. Rule 28.2 requires that the customer pay
all costs associated with the project. Under this rule, there is an estimated cost to you in the
amount of $10,541.00.

NOTE: If you have an applicable tax exemption certificate, please forward along with
this signed proposal.

If this Proposal is acceptable, please sign and return this letter. Upon receipt of your acceptance,
you will be billed under separate cover in the amount of $10,541.00. This estimate is valid for 90
days from the date of this letter.

This work will be scheduled upon receipt of this signed proposal, payment of invoice,
and completion of all responsibilities as outlined in the attached proposal.

Pursuant to PSC No. 220, you are responsible for the actual costs of this project. Upon project
completion, costs will be reconciled to the installed span footage whenever the installed span
footage varies from the footage provided in the initial design by more than 10 feet and actual
number of pole sets (if applicable). The Company will subsequently bill you if our costs run over
the invoiced amount or refund the difference if costs run below the invoiced amount. You are
responsible for all invoiced amounts.

Additionally, under Rule 28.2, there is an option to pay via a monthly surcharge for a period of 10
years (120 months). If you are interested in that option, please let me know.
